29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a.   On or about March 21, 2017, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  On or about March 21, 2017, at the above addressed jobsite, an employee performing roofing operations on a steep sloped roof, nineteen (19) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

On or about March 21, 2017, at the above addressed jobsite, employees had been provided fall protection training, however the employer failed to keep a written certification of record.
