29 CFR 1926.451(a)(1): Except as provided in paragraphs (a)(2), (a)(3), (a)(4), (a)(5) and (g) of this section, the scaffold and scaffold components were not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: a) Rangel Construction Inc. at 2280 S Downing St, Denver, CO 80210: On and before March 21, 2017, the employer did not ensure that the wood platform scaffold was designed to support, without failure, its own weight and at least 4 times the maximum intended load applied to and transmitted to it. The employer designed and installed two scaffold platforms to install soffit on a sloped roofs. One scaffold platform consisted of two 2 x 4's set at an angle and sloped from 6 feet high to approximately 10 feet high. This scaffold platform was not secured. The second scaffold platform included a 2" x 8" plank connected to one vertical support, and then to two outriggers. These designs did not ensure the scaffold components were capable of supporting at least 4 times the maximum intended load applied to support it. This condition exposed the employees to a fall hazard of up to approximately 10 feet.

Employees were installing soffit, and were not protected from falling though the use of guardrails, personal fall arrest systems, or other suitable fall protection. This condition exposed employees to fall hazards of up to approximately 10 feet.
