29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) South side of roof - On or about March 22, 2017 employees were working on top of the house attaching shingles not protected from falling with the use a fall protection system.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. (a) South side of roof - On or about March 22, 2017 employees attaching shingles to the roof has the vertical lifeline rope wrapped around the stack of shingles and not attached to an anchor point capable of holding 5,000 pounds.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. (a) North side of house - On or about March 22, 2017 employees roofing the house gained access to the roof using a "little giant tri-folding ladder" that was not tall enough to extend 3 feet above the roofs edge and it was not tied off to keep it from falling.
