On or about March 21, 2017, the employee handing materials up to the employees on the roof was not wearing a hard hat and was being exposed to head injuries.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On or about March 21, 2017, employees using nail guns to install plywood were not wearing eye protection, exposing themselves to being struck by flying debris or nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. Employees installing plywood at elevations of 6 feet or more above the ground are not being protected from fall hazards by a fall protection system:    The employer fails to assure effective protection against falls as required by the standard, as demonstrated on Mar 21, 2017, when employees installing plywood on a 5:12 pitch roof at 38 Surfside Drive, Ormond Beach, FL 32176, were exposed to an 9 feet fall hazard above the ground in that they did not have a fall protection system installed.    Roofing R US Systems, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1167770, citation number 01, item number 001 and was affirmed as a final order on February 24, 2016, with respect to a workplace located at 612 Robin Lane, Kissimmee, FL 34759.    Roofing R US Systems, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1137457, citation number 01, item number 001 and was affirmed as a final order on August 24, 2016, with respect to a workplace located at 9008 Harney Road, Tampa, FL 33637.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about March 21, 2017, front of home, employees were using a ladder that was extended 14 inches above the landing and was not secured to prevent accidental displacement.  The employees were being exposed to a 9 feet fall hazard.
