29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest systems were not designed, installed and used under the supervision of a qualified person: a) Third Story Roof - On or about March 22, 2017, the employer did not ensure that anchorages used for attachment of personal fall arrest systems were installed under the supervision of a qualified person in that a hinged roof anchor was installed with both sides along the same side of the roof ridge, thus exposing employees to fall hazards of up to about 29 feet onto gravel/dirt ground.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: a) Third Story Roof - On or about March 22, 2017, the employer did not ensure that a damaged/frayed rope was removed from service to prevent employees from using it as part of their personal fall protection system, thus exposing employees to fall hazards of up to about 29 feet onto gravel/dirt ground.
