29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a.  On or about March 22, 2017, at the above addressed jobsite, the employer did not maintain an accident prevention program to comply with the regulations of this part which included, but were not limited to employee protection when exposed to use of power tools and personal protective equipment.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a.  On or about March 22, 2017, at the above addressed jobsite, an employee conducting roofing operations was not provided training on how to minimize fall protection hazards, thereby exposing an employee to the hazards associated with falls.

On or about March 22, 2017, at the above addressed jobsite, an employee performing residential roofing operations on a step sloped roof, fifteen (15) feet above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

On or about March 22, 2017, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
