Location: a) On second floor of the building On or about 03/21/17, employees were cutting and finishing steel decking on the second floor of the building, and were exposed to falling approximately 30 feet from second floor to basement level.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. Location: a) On first floor and second floor of the building: On or about 03/21/17, employees used a grinder to cut steel decking. The grinder was connected to an extension cord, which was routed to a nearby power device and was not provided with a ground fault circuit interrupter.
