The employer is failing to protect employees from a fall hazard associated with carrying an object or load while climbing up or down a ladder. This was evident during the inspection at 1919 S. 74th St, Omaha NE where the employer had an employee climbing up and down a ladder while carrying a leaf blower to clean gutters.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment. The employer is failing to protect employees from fall hazards associated with working at heights over four feet without fall protection. The most recent occurrence of this was evident during the inspection of the facility located at 1919 S. 74th Street, Omaha, NE; where the employer had an employee without fall protection cleaning the gutters while standing on the roof more than 9 feet above the ground below. Other instances include the employee performing similar work without fall protection on the roof of 7415 Cedar St. Omaha, NE.
