29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:    On or about October 15, 2016, an employee was on top of a grain storage bin #2 acting as an observer for a bin entry. There was no guardrail system or other fall protection to protect the employee from a fall hazard of approximately 30 feet.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.272(g)(1)(ii): All mechanical, electrical, hydraulic, and pneumatic equipment which presents a danger to employees inside grain storage structures shall be deenergized and shall be disconnected, locked-out and tagged, blocked-off, or otherwise prevented from operating by other equally effective means or methods.    On or about October 15, 2016 employees entered grain bin number 2 from the top and were standing on the grain while the unload auger was running. employees were exposed to engulfment from flowing grain upon unexpected start-up of the auger.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.272(e)(1): The employer did not provide training to employees at least annually and when changes in job assignments would expose them to new hazards in the grain handling facilities:    On or about October 15, 2016 the company failed to provide adequate training at least annually on grain handling facility related hazards.  Employees were not trained to recognize hazards of working in grain handling operations including specific bin entry procedures and safe work practices applicable to their job tasks.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.272(g)(2): Whenever an employee enters a grain storage structure from a level at or above the level of the stored grain or grain products, or whenever an employee walks or stands on or in stored grain of a depth which poses an engulfment hazard, the employer shall equip the employee with a body harness with lifeline, or a boatswain's chair that meets the requirements of subpart D of this part. The lifeline shall be so positioned, and of sufficient length, to prevent the employee from sinking further than waist-deep in the grain:    On or about October 15, 2016, employees entered grain storage bin #2 from the top and were standing on the grain. The employees wore harnesses attached to a rope and tied in knots to the structure and the harnesses. The employees were exposed to an engulfment hazard due to the inadequate restraint/rescue measures including poor equipment, rope positioning and attachment.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees were not trained to recognize or eliminate electrical hazards likely to result in electrocution, shock and/or burns from contact with energized parts. 29 CFR 1910.333(b)(2): "Lockout and Tagging." While any employee is exposed to contact with parts of fixed electric equipment or circuits which have been de-energized, the circuits energizing the parts shall be locked out or tagged or both in accordance with the requirements of this paragraph. The requirements shall be followed in the order in which they are presented (i.e., paragraph (b)(2)(i) first, then paragraph (b)(2)(ii), etc.).    On or about October 15th, 2016 through December 6, 2016 the employer did not ensure verification of a safe state on circuit(s) that employees attempted to de-energize before changing fuses in enclosures up to 480 Volts.
