29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. On or about March 27, 2017, at the facility, Brut Printing Company, Inc. did not have an established program consisting of an energy control procedures, employee training and periodic inspections for machines or equipment; such as but not limited to: printing presses, letter press machines, a saddle stitcher/trimmer machine, bindery-type machines and other similar equipment, thereby exposing employees to unexpected energizing, startup or release of stored energy hazards.

On or about March 27, 2017, at the facility, Brut Printing Company, Inc. did not have a written hazard communication program while the employer allowed employees to use, store, and work in the presence of hazardous chemicals such as, but limited to blanket washes, aerosols, lubricants, cleaners, solvents and inks, thereby exposing these employees to physical and health hazards.
