On or about March 27, 2017, at the facility, Brut Printing Company, Inc. did not have an established program consisting of an energy control procedures, employee training and periodic inspections for machines or equipment; such as but not limited to: printing presses, letter press machines, a saddle stitcher/trimmer machine, bindery-type machines and other similar equipment, thereby exposing employees to unexpected energizing, startup or release of stored energy hazards.
