29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:  At 960 East Main St., Prattville, Alabama:  On or about March 23, 2017 and at times prior; the employer allowed his employees to work from a job made scaffold with a walk board which measures 14 inches wide exposing his employees to a fall hazard of 6.3 feet to the ground below.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  At 960 East Main St., Prattville, Alabama:  On or about March 23, 2017 and at times prior; the employer allowed his employees to work from a high slope roof without fall protection exposing his employees to a fall hazard of 13.2 feet to the ground below.
