29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. This violation was observed on or about March 2, 2017 at 410 Mid America Dr., Pryor, Quality Stone, Stucco, and Plastering, the employer did not ensure that employees use a ladder to ascend or descend from the scaffolding exposing employees to a fall from approximately 25 ft. to the ground below.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. This violation was observed on or about March 2, 2017 at 410 Mid America Dr., Pryor, Quality Stone, Stucco, and Plastering did not ensure each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the JLG Aerial lift exposing employees to a fall from approximately 25 ft. to the ground below.
