29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a.  On or about March 23, 2017 two employees were installing plywood sheathing on a new 6:12 pitch steep slope residential roof and were not protected by any means of personal fall protection, guardrails, or safety nets, exposing them to falls in excess of 9 feet to the ground below.

29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable.  a. On or about March 23, 2017 the employer did not assure employees using pneumatic nail guns were protected from impact of flying objects to the eyes, by the use of safety glasses or goggles. One employee was observed installing roof sheathing and truss blocking using an Hitachi Pneumatic Nail Gun and was not wearing safety glasses, or goggles.
