29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: a) Plant - On or about 03/20/17 and at times prior thereto employees were instructed to perform set-up, maintenance, and repairs of equipment including (but not limited to) the Lyle 152 PV trim press. Fall protection equipment was not in use, and workers were exposed to falls from an approximate elevation of 5.0 ft. 10.0 in. onto the concrete floor. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: a) Inline Plastics Corp. - On 02/27/17 an employee was hospitalized with severe head injury and rib fractures after fall from machinery. The employer failed to report the hospitalization to OSHA. No abatement action is required for this item.
