29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off, and brakes set. Wheels shall be blocked if the truck is parked on an incline.    Location: Jobsite    Employees were exposed to crushing and struck by hazards while working below an industrial rough terrain fork truck that was left unattended with boom and load extended to the second floor level of the building, while employees were unloading a pallet of gypsum wallboard panels.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee.    Location:  Jobsite    Employees were exposed to fall hazards where anchorage points were not capable of supporting at least 5,000 pounds per employee where:    Instance 1)  Employees had used choker straps wrapped through wooden parallel chord trusses with light gauge metal gusset plates that were incapable of supporting lateral forces and loads.     Instance 2)  Nails were used in lieu of 3" long #12 hex head screws in fastening down hinged roof anchors

When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours.    Location: Jobsite    Employer failed to provide logs to OSHA in the time required by the standard. Mauro Bieanse state through his brother(interpreting) that this company chooses not to keep OSHA 300 Logs.
