The unused portion of the blade was not guarded for the portion of the blade between the sliding guide and the upper saw wheel. The unused portion of the blade was not guarded under the table.     b.  On or about March 22, 2017, the employer failed to ensure that the W.F. Wells 9" Metal Saw, Model M horizontal band saw, was guarded. The unused portion of the blade was not guarded.    c.  On or about March 22, 2017, the employer failed to ensure that the Lang LGR Engraver #1 was guarded. The platform table was not guarded.    d.  On or about March 22, 2017, the employer failed to ensure that the Powermatic Model 1200 drill press was guarded. The rotating chuck was not guarded.    e.  On or about March 22, 2017, the employer failed to ensure that the Monarch VMC45B Vertical CNC was guarded. There were no interlocks.    f. On or about March 22, 2017, the employer failed to ensure that the Monarch (model unknown) Vertical CNC was guarded. The interlocks were not functioning.    g.  On or about March 22, 2017, the employer failed to ensure that the Makino GF8-1 vertical machining table was guarded. There were no interlocks.    h.  On or about March 22, 2017, the employer failed to ensure that the Cincinnati Boring Mill 12' was guarded. The platform table was not guarded.    i.  On or about March 22, 2017, the employer failed to ensure that the Giddings & Lewis 5219 was guarded. There were no interlocks.    j.  On or about March 22, 2017, the employer failed to ensure that the Wotan Cutter was guarded. The cutter needs guarded.    k.  On or about March 22, 2017, the employer failed to ensure that the South Bend lathe was guarded. There was no chuck guard.    l.  On or about March 22, 2017, the employer failed to ensure that the Springfield lathe was guarded. There was no chuck guard.
