29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: (a) On or about February 14, 2017, the exposing employer, A&V Roofing and Construction, did not ensure employees, who were required to engage in roofing maintenance operations at a residential apartment complex, were protected from exposure to fall hazards in excess of 25 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
