29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: Garage Area - The power cord for the mobile column lifts contained a ¼" hole that penetrated the outer jacket of the cord and had not been removed from service to protect employees from potential shock hazards.
