29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: a) 1311 Northwest Blvd, Building 351: On or about March 23, 2017, and times prior thereto, an employee that removed a walkway railing to allow materials to be landed, with an approximately 20' walkway to ground fall distance, and an approximately 6' walkway to second floor steel and masonry railing fall distance, used a railing for anchorage, that was not designed, installed and used as part of a complete personal fall arrest system that maintained a safety factor of a least two; and under the supervision of a qualified person, exposing the employee to fall hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 1311 Northwest Blvd, Building 351: On or about March 23, 2017, and times prior thereto, an employee that removed a walkway railing to allow materials to be landed, with an approximately 20' walkway to ground fall distance, and an approximately 6' walkway to second floor steel and masonry railing fall distance, used an MSA 6' diamond lanyard attached at ankle level to a fixed walkway railing, rigged such that the employee could free fall greater than 6 feet, exposing the employee to fall hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
