29 CFR 1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.     a. On or about March 23, 2017 - at the above addressed jobsite, employees were required to access / egress a residential roof using a ladder placed in close proximity to the electrical utility service drop. Employees were thereby exposed to electrical burns / shock.    b. Employees were observed working above or around the electrical utility service drop at the time of inspection. Employees were thereby exposed to electrical burns / shock.    No abatement is required for this item.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     On or about March 23, 2017, employees performing residential roofing installation operations at a residential property were exposed to a 8' foot fall hazard when the employer failed to ensure fall protection was utilized.      No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support:     a.  On or about March 23, 2017 - at the above addressed jobsite, employees were required to work from a ladder that did not extend three (3) feet above the roof's eve during roofing material removal / installation operations at heights up to eight (8) feet. Employees ascending / descending the ladder were thereby exposed to fall hazards.     No abatement certification or documentation is required for this item.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support):    a.  On or about March 23, 2017 - at the above addressed jobsite, employees were required to work from ladders during roofing material removal / installation operations while ascending / descending a ladder placed at an unsafe angle.
