29 CFR 1926.1052(b)(2): Foot traffic was not prohibited on skeleton metal stairs where permanent treads and/or landings were to be installed at a later date and the stairs were not fitted with secured temporary treads and/or landings long enough to cover the entire tread or landing area:   a)  Landings, Stairway No. 1  On or about March 21, 2017, the 7 foot 2 inch by 3 foot 9 inch pan stair landings in a stairway, which were not under construction and provided for temporary use, were not fitted with temporary landings thereby exposing workers to a potential 10 ft. fall hazard.  b)  Landings, Stairway No. 2  On or about March 21, 2017, the 7 foot 2 inch by 3 foot 9 inch pan stair landings in a stairway, which were not under construction and provided for temporary use, were not fitted with temporary landings thereby exposing workers to a potential 10 ft. fall hazard.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail:    a)  Stairway No. 1  On or about March 21, 2017, flights of metal stairs, having at least nine risers and which ran between four floors of a building under construction, were not provided with handrails thereby exposing workers to a potential 10 ft. fall hazard.   b)  Stairway No. 2  On or about March 21, 2017, flights of metal stairs, having at least nine risers and which ran between four floors of a building under construction, were not provided with handrails thereby exposing workers to a potential 10 ft. fall hazard.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1926.451(b)(3): The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:   a)  Atrium  On or about March 21, 2017, two workers installing sheathing and insulation boards from the 12 inch wide platform of a pump jack scaffold were exposed to a 40 foot fall for their work platform was located 21 inches (horizontally) away from the wall surface they were working upon.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a)  Atrium  On or about March 21, 2017, four workers installing sheathing and insulation boards from the platform of a pump jack scaffold were not provided with personal fall protection and were exposed to a 40 foot fall.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
