29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  On or about March 21, 2017, the employer, Auto Crushers, Inc., did not establish an energy control program so as to protect employees who were exposed to the unexpected energizing, startup or release of stored energy while engaged in the servicing or maintenance of the Overbuilt Model 10 Car Crusher. 29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:  a)  On or about March 21, 2017, and at times prior, employees were exposed to hazardous chemicals in that containers, located near the tank station, were inadequately labeled.        1) Four 50 gallon drums of windshield washer fluid were not labeled.        2) One large drum containing used motoroil was not labeled.
