29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  Automatic Transfer Switch Panel in Building 3015: On March 24, 2017, employees were not familiar with all elements of the Electrical Safe Program. Employees were working in the presence of energized electrical components without de-energizing and/or did not apply individual locks in a group-lockout operation. Employee(s) were exposed to electrical shock hazards up to 480 volts.

29 CFR 1910.333(a)(1): Live parts to which an employee may be exposed were not de-energized before the employee worked on or was near to them:  Automatic Transfer Switch Panel in Building 3015: On March 24, 2017, energized component(s) inside the panel, such as but not limited to the bus bar, were not de-energized prior to an employee working on the extension cables from the permanently mounted generator.  Employee(s) were exposed to electrical shock hazards up to 480 volts.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to de-energize circuits and equipment on which work was to be performed:  Automatic Transfer Switch Panel in Building 3015: On March 24, 2017, each authorized employee working at the main breaker panel did not affix a personal lockout/tagout device prior to performing work, such as but not limited to landing cables, at the open panel. Employee(s) were exposed to electrical shock hazards up to 480 volts.
