NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft:    a) Organic Room: A procedure which afforded the employees a level of protection equivalent to that provided by the implementation of personal lockout or tagout device was not utilized by production and maintenance employees that disassemble and resembe the Stuffer and Chub Cutter Machine when cleaning. A group lockout device was not used during this operation, on or about 2/25/17.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Saw Department: The unused portion of the Hollymatic band saw blade was not guarded when cutting various size pieces of beef. On or about 6/1/17.    b) Saw Department: The unused portion of the three Biro band saw blades were not guarded when cutting various size pieces of beef. On or about 6/1/17.
