On or about March 22, 2017 and times prior thereto, at the worksite located at or near 110 Bayonne Cove in Maumelle, Arkansas, employees were observed using pneumatic nail guns to install new shingles on an existing residential house without any eye protection. This condition exposed employees to struck-by hazards.

The condition exposed employees to fall from height hazards.

On or about March 22, 2017 and times prior thereto, at the worksite located at or near 110 Bayonne Cove in Maumelle, Arkansas, employees were observed replacing shingles on a residential house over 6 feet high were using rock-climbing harnesses that attached in the front, middle of the employee's body. The condition exposed employees to lack of a proper fall arrest system.
