29 CFR 1910.178(l)(2)(ii):     The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    The employer is failing to protect employees from a struck by hazard by not ensuring each operator has successfully completed formal training, practical training, and a performance evaluation prior to operating a forklift.  This was observed at 10308 S 144th St, Omaha, NE 68137 where forklift operators did not receive any training before operating a forklift.

A flexible cord coming from the employee sign-in box was spliced with another cord and both ends went to two different outlets.
