On 3/15/17, a production employee was in the process of wiping down an inflatable slide with a rag containing acetone and ethyl acetate blend and was injured when the rag ignited while in their right hand.      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
