29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.    a) On or about March 22, 2017: Employees were using an extension ladder to access the roof, which was place in the front of the building on a walking area next to the entrance to the dwelling units and the ladder was not secured to prevent it from displacement.

29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye injury.    a) On or about March 22, 2017:  two employees operating nail guns to install roof shingles were not wearing safety glasses, the employees were exposed to eye injuries.    Walker Brothers Enterprises, Inc. dba Walker Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1130188, citation number 1, item number 1 and was affirmed as a final order on March 18, 2016, with respect to a workplace located at 1619 S. Peninsula Drive Daytona Beach, FL 32114.

Walker Brothers Enterprises, Inc. dba Walker Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1130188, citation number 1, item number 2 and was affirmed as a final order on March 18, 2016, with respect to a workplace located at 1619 S. Peninsula Drive Daytona Beach, FL 32114.
