29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about March 27, 2017, at the site,  employees were performing servicing and/or maintenance activities that included unjamming machines with multiple energy sources without the employer developing energy control procedures. Employees exposed to an amputation or electrical shock from the unexpected start-up of equipment.  a) Mosley brass and metal stripping machine, there are no energy control procedures developed.  Employee(s) were exposed to being caught-in the machine and/or electrical shock. The Mosley brass and metal stripper has as a minimum of electrical and hydraulic energy.   b) Sierra Logger crusher, there are no energy control procedures developed. Employee(s) exposed to being caught-in the machine and/or crushed. The Sierra Logger crusher has a minimum of combustion, electrical and hydraulic energy.    c) REM can crusher, there are no energy control procedures developed. Employee(s) exposed to being caught-in the machine and/or crushed. The REM can crusher has a minimum of electrical and hydraulic energy.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  On or about March 27, 2017, the employees were not trained to have the knowledge and skill necessary for the safe application, usage and removal of energy control devices when maintaining, cleaning, unjamming equipment listed below. Employees exposed to an amputation or electrical shock hazard from the unexpected start-up of equipment.   a) Mosley Metal stripper for brass. b) Sierra Logger metal crusher. c) REM can crusher.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about March 27, 2017, the Mosley brass/metal stripper was not guarded. Employees operate this machine to strip brass off junk to reclaim it. Employees exposed to the hazard of an amputation.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about March 27, 2017, the employer failed to ensure the employees were trained on hazardous chemicals required to be used in the work place such as, but not limited to:   a) Gunk Degreaser, manufactured by RSC Chemical Solutions. b) WD-40, manufactured by WD-40 Company.  c) Macs 1726 Premium Starting Fluid, manufactured by NAPA.
