29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms and walking-working surfaces were not kept clean, orderly and in sanitary condition: CNC AREA On, or about April 17, 2017, the employer failed to keep the CNC areas free of accumulated combustible saw dust. Saw dust generated from the Busellato Jet Pulse and the NVANDI NC-1918PT CNC Machine had accumulated on electrical conduit, wires, walking-working surfaces around the machines and on and around the motor used to energize the 250lb Gorbel Fisher vacuum lift.

29 CFR 1910.1052(k)(1)(iii): The employer did not include methylene chloride in the hazard communication program established to comply with hazard communication standard (HCS) (1910.1200) and did not ensure each employee was trained in accordance with the requirements of HCS and paragraph (l) of this section: LAMINATE On, or about, April 27, 2017, the employer failed to include methylene chloride in the hazard communication program and did not train each employee on the hazards associated with methylene chloride, medical surveillance requirements and additional requirements of paragraph (l), when employees were exposed to methylene chloride while working with chemicals including, but not limited to, EverStrong ES130 Contact Adhesive, which contained 80-100 percent methylene chloride, used on wood laminate.
