29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    (a)  PRODUCTION AREA, PRESS ROOM - Employee(s) performed the set-up for Press 104, and all of the employee(s) performing the work on the machine had not attached their own individual lock for lock out purposes, thereby exposing the employee(s) to serious injury, on or about March 23, 2017.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  PRODUCTION AREA, PRESS ROOM - Employee(s) operating the Waldren Surface Printing Presses were exposed to rotating parts of the press when they used a scrub pad to clean debris from the rubber press wheel.  An employee performing this task had his hand and arm drawn in-between the central impression wheel and an idler shaft, on or about March 20, 2017.    No abatement certification or documentation required.
