29 CFR 1926.1053(b)(9): The area around the top and bottom of ladders shall be kept clear. a) David Trevino Dba Trevino Roofing Construction project site on the 5000 Block of Pennsylvania Ave. Appleton, WI: On or about March 27, 2017 the area around the bottom of the portable extension ladder was clutter with debris and shingle pieces creating a tripping hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) David Trevino Dba Trevino Roofing Construction project site on the 5000 Block of Pennsylvania Ave. Appleton, WI: On or about March 27, 2017 five employees engaged in roofing work were not protected from a 10 ft. fall hazard from the roof eave to ground level. Employees were wearing full body harnesses but were not connected by lanyards to anchor points installed on the roof. David Trevino Dba Trevino Roofing Construction was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection Number 943523, citation number 1, item number 2 and was affirmed as a final order on February 6, 2014, with respect to a worksite located on Pennsylvania Avenue, Appleton, WI.
