29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    a) Shipping Receiving Area- On March 24, 2017, the employer failed to ensure that all powered industrial trucks with modifications and additions, which affect the capacity, were performed with the written approval of the manufacturer and all capacity plates were changed accordingly.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a) Shipping Receiving Area- On March 24, 2017, the employer failed to ensure that all powered industrial trucks equipped with front-end attachments other than the factory installed forks, such as the 72 inch Vestil Fork Extension Caps on the Yale Forklift, were provided markings to identify the attachments and the approximate capacity of the attachment combination and truck with the load laterally centered.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) Shipping Receiving Area- On March 24, 2017, the employer failed to ensure that each powered industrial truck operator was competent to operate powered industrial trucks, such as the Yale Forklift, safely as demonstrated by the successful completion of operator training and evaluation. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) Bindery Area- On March 24, 2017, the employer failed to ensure that electrical equipment, such as the multiple outlet relocatable power taps in the Bindery area, were being used only to power audiovisual equipment as permitted under its listing and labeling.  The multiple outlet relocatable power taps were used to power additional multiple outlet relocatable power taps, the Automatic Paper Folder, the Duplo Booklet Maker and other extension cords.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
