OR IN THE ALTERNATIVE:    29 CFR 1910.147(d)(4)(I): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    (a) Facility, 2341 Plank Road, Duncansville, PA:  On or about March 17, 2017 lockout/tagout devices were not affixed to each isolating device by an authorized person when a servicing and/or maintenance task was being performed.
