29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility, 2341 Plank Road, Duncansville, PA: On or about March 17, 2017 there was no guard in place to prevent contact with the moving parts of the motor. An employee was checking a vacuum for suction when they made contact with the moving parts of one of the motors sustaining an amputation of the tip of their right middle finger. OR IN THE ALTERNATIVE: 29 CFR 1910.147(d)(4)(I): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Facility, 2341 Plank Road, Duncansville, PA: On or about March 17, 2017 lockout/tagout devices were not affixed to each isolating device by an authorized person when a servicing and/or maintenance task was being performed.
