29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the jobsite,  located at 9620 Rose Hill Dr., Lenexa, Kansas employees engaged in framing activities, working and walking on the second floor of the building structure, were exposed to fall hazards of approximately 13 and 1/2  feet from the second floor of the structure to the ground below, in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:  At jobsite located at 9620 Rose Hill Dr., Lenexa, Kansas, employees using the D6224-2 24 feet fiberglass WERNER ladder to access the working level were exposed to fall hazards of up to approximately 13 1/2 feet to the ground below in that the ladder did not extend at least 3 feet above the landing surface.  The ladder extended approximately 2 1/2 feet.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours:  At the jobsite, locates at 9620 Rose Hill Dr. Lenexa, Kansas, the employer failed to provide copies of the injury and illness logs to the OSHA Compliance Safety and Health Officer.  Copies of the OSHA 300 logs, and 300A summaries for 2012, 2013, 2014, 2015 and 2016 were not provided to OSHA within four business hours of their request.
