29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Front of house, on or about March 28, 2017, an employee working below an area where the installation of siding was being performed, was not wearing a hard hat, exposing the employee to a head injury.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   A) Front of building on or about March 28, 2017, an employee was working from a ladder jack scaffold without the use of fall protection equipment, exposing the employee to a fall hazard of 19 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  A) Front of house, on or about March 28, 2017, employees worked from the roof of a porch without fall protection to perform siding work, exposing the employees to a fall hazard of approximately 16 feet.
