Worksite located at 1801 Amesbury Ave., Liberty, Missouri- Employees were exposed to a fire or explosion hazard in that flammable liquids were being dispensed without the use of a metal safety can or a DOT approved container.

Worksite located at 1801 Amesbury Ave., Liberty, Missouri- Employees were exposed to fall hazards while using a ladder that was not secured and was not extended at least 3 feet above the upper landing surface.
