29 CFR 1926.761(a):  Training required by this section was not provided by a qualified person(s):      The employer failed to provide a training program for employees performing steel erection activities that could be exposed to fall hazards.  This violation was most recently documented at a worksite located at or near Harney Street and South 204th Street, Elkhorn, Nebraska 68022 on March 20, 2017. Employees were performing steel erection activities on a roof and they had not received training in accordance with the steel erection standards.      Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.

29 CFR 1926.1053(b)(13): The top or step of a stepladder was used as a step:    The employer failed to ensure that employees were using a A-frame ladder for the purpose in which it was designed exposing the employees to the hazards of falls.  This violation was most recently documented at a worksite located at or near Harney Street and South 204th Street, Elkhorn, Nebraska 68022 on March 20, 2017. Employees were using a portable A-frame ladder to access an upper landing surface while building a steel storage shed.  The ladder was not the correct height to access the roof without using the top two steps for access which is prohibited by the standard.       Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways:                                                                                            The employer failed to ensure that employees were trained to recognize hazards related to ladders or trained in the procedures to be followed to minimize these hazards. Employees were using a portable A-frame ladder to access an upper landing surface while building a steel storage shed.  The ladder was not high enough for employees to access the upper landing surface without using the top two steps which is prohibited. This violation was most recently documented at a worksite located at or near Harney Street and South 204th Street, Elkhorn, Nebraska 68022 on March 20, 2017.    Abatement certification is required for this violation.  Abatement certification sheet is enclosed with the citation.
