Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed by hazards: a. 21 Henderson Drive #2 West Caldwell, NJ (LTS Lohman Therapy Systems Corp Collection Site) The employer does not ensure written operating procedures covering the steps for loading and unloading roll off containers provided clear instructions for safely conducting such activities; there by exposing operators to struck-by and crushing hazards.
