29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Worksite: The employer allowed two employees to work using a stack of plywood sheets as a scaffold platform, and the stack was not attached to the forks to prevent movement of the platform while the employees were working on it.
