29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) The employer failed to ensure that the Amada Press Brake number five (5) (Model No. HFE220-4s, Serial No. HFE220-4 S N070264) was properly guarded to prevent employees from accessing the point of operation during the operational cycles.  This exposed employees to amputation injury from caught-in and pinch hazard created by the press brake dies point of operation.     The Alto-Shaam, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 670178, citation number 1, item number 1 and was affirmed as a final order on 12/19/2012, with respect to a workplace located at W164 N9283 Water Street, Menomonee Falls WI 53051.
