29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:  a) South side of building:  On or about March 28, 2017 and at times prior thereto employees performing re-roofing activities on a 16 in 12 pitched roof, positioned tubular welded scaffold systems at various locations along the roofs eave.  The scaffold was used as a flat work surface for the roofing work above.  A guardrail or other fall protection system was not in place, exposing the employees to fall hazards up to 19 and a half feet to the ground below.    Note) Certification of corrective action is required for this item.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:  a) North side of building: On March 28, 2017 and at times prior thereto an employees was staging roofing materials on a flat roof deck.  A warning line or another type of fall protection system was not in place.  The employee was exposed to a 13 foot fall to the ground below. 29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  a) North side of building: On or about March 28, 2017 and at times prior thereto employees performed re-roofing activities on a 16 in 12 pitch roof without a fall protection system in place. Employee(s) were exposed to fall hazards ranging from approximately 11 feet to the eave/scaffold platform below, up to an estimated 40 feet from the roofs rake edge to the ground below.
