29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) Roof - On or about March 29, 2017 employees were not wearing impact resistant eye protection while using a leaf blower and pneumatic nail guns to install felt paper on the roof of a residential home, thus exposing employees to possible eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Roof - On or about March 29, 2017, employees installing new shingles on an existing one-story residence were exposed to fall hazards up to approximately 11 feet onto earthen ground because the workers were not fall protected.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite - On or about March 29, 2017 and times prior to, employees were not instructed in the hazards associated with their work duties including but not limited to fall protection requirements when working on residential roofs, thus exposing employees to fall hazards up to about 11 feet.
