29 CFR 1910.1001(g)(1): The employer did not provide respirators, and ensure that they were used, where required by this section: Emory University-Atlanta, GA-Throughout the facility (i.e. Steam tunnels, boilers, manholes, basement, and mechanical rooms)-Employees were exposed to lung damage, cancer, and skin irritation hazards when required to work in areas that containing Thermal System Insulation (TSI) , such as, but is not limited to valves, pipes, flanges, insulation, and steam lines. Employees work included, but was not limited to grinding, repairing and replacing assumed asbestos containing material (ACM). Sample results after grinding valves, gaskets, and flanges with pneumatic tools confirmed asbestos containing material (ACM). Respirators for this maintenance and repair activity with unknown concentrations of asbestos were not assigned or required, on or about 03/27/17. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.1001(h)(1): The employer did not ensure that employees exposed to asbestos at concentrations greater than the TWA and/or excursion limit, or where the possibility of eye irritation exists, wore the work clothing and protective equipment provided: Emory University-Atlanta, GA-Throughout the facility (i.e. Steam tunnels, boilers, manholes, basement, and mechanical rooms)-Employees are exposed to skin/eye irritation, when required to work in areas that containing Thermal System Insulation (TSI) , such as, but is not limited to valves, pipes, flanges, insulation, and steam lines. Sample results after grinding valves, gaskets, and flanges with pneumatic tools confirmed asbestos containing material (ACM). Employees work included, but was not limited to grinding, repairing and replacing assumed asbestos containing material (ACM). Coveralls, eye protection, etc. for this maintenance and repair activity with unknown concentrations of asbestos while working with ACM were not assigned or required, on or about 03/27/17. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. Employees work included, but was not limited to grinding, repairing and replacing assumed asbestos containing material (ACM). The employer had not identified and or tested these areas prior to assigning and performing such work for maintenance and repair, on or about 03/27/2017. Sample results after grinding valves, gaskets, and flanges with pneumatic tools confirmed asbestos containing material (ACM).
