29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  Instance a): 110 Main Street, East Brunswick, NJ 08816. Employee was exposed to eye and face injuries from flying debris when operating a Makita Circular saw to cut plywood roof sheathing while not wearing any eye and or face protection.   Instance b): 110 Main Street, East Brunswick, NJ 08816. Employee was exposed to eye and face injuries from flying debris when operating a Hitachi pneumatic nail gun to install plywood sheathing while not wearing any eye and or face protection.   Occurred on 03/23/17.

29 CFR 1926.501(b)(11): Each employee on a steep roof (greater than 4/12 pitch) with unprotected sides and edges 6 feet or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) 110 Main Street, East Brunswick, NJ 08816. Employee was exposed to fall hazards in excess of 15 foot from the eave to the ground below while installing plywood roof sheathing with no fall protection systems in place.   b) 110 Main Street, East Brunswick, NJ 08816. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  a) 110 Main Street, East Brunswick, NJ 08816. Employee was exposed to fall hazards in excess of 18 feet from the landing to the ground below when the ladder he was using to access the rear roof did not extend 3 feet above the landing.   Occurred on 03/23/17.
