Causeway Approach, Mandeville, LA, the employer failed to ensure that the scaffold system was fully planked. Employees were utilizing the scaffold system to apply stucco to the arch of the building.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. On or about March 24, 2017, at 1200 W. Causeway Approach, Mandeville, LA, the employer failed to ensure that the ladder extended past the second level. The scaffold consisted of four working levels. Employees utilized the cross braces in order to access the third and fourth levels.

For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall. An employee was walking on the second level while work was being performed overhead.
