29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) Roof - On or about March 28, 2017, employees installing new shingles on an existing one-story residence were exposed to fall hazards up to approximately 10 feet onto earthen ground because the workers were not fall protected.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite - On or about March 28, 2017 and times prior to, employees were not instructed in the hazards associated with their work duties including but not limited to fall protection requirements when working on steep roofs (roof pitch greater than 4/12), thus exposing employees to fall hazards up to 10 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Roof, Front Elevation - On or about March 28, 2017, employees used an aluminum extension ladder to gain access to the roof of the building without the side rails of the ladder being extended at least 3 feet above the upper landing surface, exposing employees to fall hazards of up to approximately 10 feet onto earthen ground.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a) Roof, Front Elevation - On or about March 28, 2017, employees were carrying 50-pound bags of shingles while ascending an aluminum extension ladder to gain access to the roof, which could cause employees to lose balance and fall, thus exposing employees to fall hazards of up to approximately 10 feet onto earthen ground.
