29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On the Mark Andy MA two (2) machine, the employer did not ensure that the control of hazardous energy procedure was used during servicing and maintenance activities including, but not limited to, cleaning and set-up operations, exposing employees to pinch points, caught-in-between hazards and amputation injuries. (b) On the Mark Andy MA three (3) machine, the employer did not ensure that the control of hazardous energy procedure was used during servicing and maintenance activities including, but not limited to, cleaning and set-up operations, exposing employees to pinch points, caught-in-between hazards and amputation injuries. (c) On the Mark Andy MA four (4) machine, the employer did not ensure that the control of hazardous energy procedure was used during servicing and maintenance activities including, but not limited to, cleaning and set-up operations, exposing employees to pinch points, caught-in-between hazards and amputation injuries. (d) On or about March 23, 2017, an employee performing cleaning of a roller on the Mark Andy MA five (5) machine was exposed to pinch points, caught-in-between hazards and amputation injuries, in that the employer did not ensure that the control of hazardous energy procedure was used to prevent inadvertent starting of the machine.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On the Mark Andy MA two (2) Machine, the employer did not ensure that moving components, including, but not limited to gears, rollers and spindles were guarded, exposing employees to amputation injuries. (b) On the Mark Andy MA three (3) Machine, the employer did not ensure that moving components, including, but not limited to gears, rollers and spindles were guarded, exposing employees to amputation injuries. (c) On the Mark Andy MA four (4) Machine, the employer did not ensure that moving components, including, but not limited to,gears, rollers and spindles were guarded, exposing employees to amputation injuries. (d) On the Mark Andy MA five (5) Machine, the employer did not ensure that moving components, including, but not limited to gears, rollers and spindles were guarded, exposing employees to amputation injuries.
