(a) Tomco Tree Service LLC on or about March 30, 2017, at the address of 415 S. Main St. Wellington, OH: The employer did not ensure an employee performing tree trimming operations from an elevated vehicle mounted platform, utilized an appropriate fall protection system. The employee performing work in this condition was exposed to an approximant 40 ft. fall to the ground below.

Main St. Wellington, OH: The employer did not ensure the eyes of an employee operating the chainsaws onsite were protected from flying wood chips/debris: Tomco Tree Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.133(a)(1), which was contained in OSHA inspection number 1087234, citation number 1, item number 3 and was affirmed as a final order on October 21, 2015, with respect to a workplace located on Lake Rd. Rocky River, OH.
