In the mixing room (Rockwell Hitec HV3 automatic chemical  mixer), the floors were covered with spilled chemicals, and discarded materials. The employer did not ensure that the facility was kept clean and in orderly condition. In the mixing room (Rockwell Hitec HV3 automatic chemical  mixer), employees use and are exposed to corrosive chemicals, such as, but not limited to, Sodium Hydroxide. The employer did not provide a suitable facility for flushing of the eyes. The condition was noted at or around 03/29/17.
